How to Write a Nursing Care Plan for Canadian Programs

A Canadian nursing care plan has five parts: assessment data, a nursing diagnosis in NANDA format, measurable goals, planned interventions with rationales, and evaluation criteria. The rationale column is where most marks are won or lost, because it proves the intervention is evidence based.
Write the diagnosis as problem, related factor, and evidence. For example: impaired gas exchange related to alveolar capillary membrane changes as evidenced by oxygen saturation of 88 percent on room air.
Goals must be measurable and time bound. Patient will maintain oxygen saturation above 92 percent on room air within 48 hours is markable. Patient will breathe better is not.
Every intervention needs a cited rationale from current evidence, ideally within five years, and Canadian sources where available: CNA position statements, provincial college standards, RNAO best practice guidelines.
Evaluation closes the loop. State whether the goal was met, partially met, or not met, and what you would change. Students who skip this section routinely lose ten to fifteen percent.
